  10. 19________. I, Kevin Wallaby, being of sound mind, willfully and
  11. voluntarily make known my desires that my dying shall not be
  12. artificially prolonged under the circumstances set forth below, do
  13. hereby declare:

  15.     If at any time I should have an incurable injury, disease, or
  16. illness certified to be a terminal condition by two physicians who
  17. have personally examined me, one of whom shall be my attending
  18. physician, and the physicians have determined that my death will
  19. occur whether or not life- sustaining procedures are utilized and
  20. where the application of life-sustaining procedures would serve
  21. only to artificially prolong the dying process, I direct that such
  22. procedures be withheld or withdrawn, and that I be permitted to
  23. die naturally with only the administration of medication or the
  24. performance of any medical procedure deemed necessary to provide
  25. me with comfort care.

  27.     If, in spite of this declaration, I am comatose or otherwise
  28. unable to make treatment decisions for myself, I  HEREBY designate
  29. Sheila Wallaby to make treatment decisions for me.

  31.     In the absence of my ability to give directions regarding the use
  32. of such life-sustaining procedures, it is my intention that this
  33. declaration shall be honored by my family and physicians as the
  34. final expression of my legal right to refuse medical or surgical
  35. treatment and accept the consequences from such refusal.

  37.     I understand the full import of this declaration and I am
  38. emotionally and mentally competent to make this  declaration.
